Rare Presentation of Cardiac Sarcoidosis With Recurrent Large Pericardial Effusion and Stress-induced Cardiomyopathy

A 65-year-old female known to have type 2 diabetes, with no significant past medical history, had presented another hospital a 2-week history of progressive dyspnoea, orthopnoea and, lower limb oedema. Echocardiography revealed large pericardial effusion echocardiographic signs increased intrapericardial pressure. An emergency pericardiocentesis was performed. After 1 week, the patient admitted King Fahd Military Medical Complex (Dhahran, Saudi Arabia) because re-accumulation effusion, which mandated pericardiocentesis. CT chest enlarged mediastinal lymph nodes. Video-assisted thoracic surgery performed obtain window and biopsy from nodes, non-caseating granuloma, highly suggestive sarcoidosis. few days after experienced neurogenic bladder, acute renal shutdown, metabolic acidosis, during she developed stress-induced cardiomyopathy that improved later. The patient’s symptoms within weeks receiving corticosteroids.
